I AGREE TO PAY: In full for drain cleaning/snaking service/s requested regardless of the outcome of the services rendered to clear my drain system. This includes, drain treatment, locating of unknown mainline drains, jetting, camera inspection, or pulling toilets due to issues beyond snaking.

HOLD HARMLESS: In requesting the services indicated. I understand that the condition of the drains and plumbing fixtures may have deteriorated due to age, exposure to acid or other chemicals, or normal wear and tear. Therefore, in consideration of the services requested, I hereby release GEE'S DRAIN ROOTER SERVICE LLC, from any claims of property damage, including consequential damages, in connection with or arising out of the services requested, I further agree to indemnify and hold GEE'S DRAIN ROOTER SERVICE LLC, harmless against any such claims made by any third party.
